Before School and After School Care
Below are links to the forms to enrol and book for our Before School and After School Care Programs.
All families are required to fill in a new Booking Form every year. Previous years' booking forms do not "roll over".
Submission of forms and processing time
Please note that for new enrolments that there is a two day processing period from the date of receipt of all complete information and the first booking commencing. This is to ensure that staff at each service have all the information required to meet both regulations and to be able to contact you if needed.
Forms to complete
Completed forms may be returned by mail, fax, email or in person to Y Central.

If any details in the original Enrolment or Direct Debit Payment Form have changed, please fill an
Enrolment Information Update Form or new
Direct Debit Payment Form.
