Coronavirus Situation Update

As of Wednesday, 15th April 2020, all our Ballarat OSHC services with the exception of Lucas, are operating to provide care for children of essential service workers and for vulnerable students.

Before School and After School Care

Below are links to the forms to enrol and book for our Before School and After School Care Programs.
All families are required to fill in a new Booking Form every year. Previous years' booking forms do not "roll over".

Forms to complete             

​ ​ STEP 1:  Enrolment* 

All Venues
ALL new families are required to fill in a new Enrolment Form prior to the commencement of Before School Care, After School Care or a School Holiday Program.

* This form only needs to be completed by each family on initial enrolment.
   Should details change, pl​ease use the Enrolment Information Update Form.

STEP 2:  Complete Payment Details*​

​​Payment - Direct Debit Form
Payment for all our Before School and After School Care programs is by Direct Debit.

* The Direct Debit Form applies to Before and After School Programs services and only needs to be
  completed by each family once or when details change.

​ ​STEP 3:  Complete Booking Form

​​A new booking form is required to be completed each year by all families.

​Submission of forms and processing time

Please note that for new enrolments that there is a two day processing period from the date of receipt of all complete information and the first booking commencing. This is to ensure that staff at each service have all the information required to meet both regulations and to be able to contact you if needed.

Updating & changing information

If any details in the original Enrolment or Direct Debit Payment Form have changed, please fill an
Enrolment Information Update Form or new
Direct Debit Form.
